I have less outbursts so I think it helps with that. I am currently on 1 mg 3x daily. There is no kick to it, in that it's so smooth I'm not sure it's working. I've been on this medicine for about a month.

Bipolar disorder is marked by episodes of mania () and hypomania (), and nearly always Benzodiazepines — We suggest clonazepam for patients who have.

Severe manic episodes — Severe manic or mixed episodes are characterized by suicidal or homicidal ideation or behavior, aggressiveness, psychotic features (ie, delusions or hallucinations), and poor judgement that places the patient or others at imminent risk of being harmed. Severely ill patients generally require hospitalization.

A standardized rating scale, such as the Young Mania Rating Scale, can be used to quantify response, although this is not standard clinical practice. For patients who do not achieve remission, a reasonable goal is response, which is defined as stabilization of the patient’s safety and substantial improvement in the number, intensity, and frequency of mood (and psychotic) symptoms.

Electrode placement and other aspects of ECT technique for treating mania have not been standardized.

The author summarizes studies done by the Bipolar Research Group of the Clinical Psychopharmacology Unit at Massachusetts General Hospital. Although most of the research work to date has involved carbamazepine, improvement in bipolar symptoms with clonazepam has been exhibited in controlled studies and case reports. The use of anticonvulsants as treatments for bipolar affective disorder is growing, and despite relatively scant data, anticonvulsants have become widely accepted as adjuncts to lithium therapy, particularly as alternatives to neuroleptics. The data thus far indicate that clonazepam can be added to the treatment regimen of bipolar patients and apparently reduces cycle frequency; many patients using neuroleptics and lithium can be switched to lithium and clonazepam without suffering acute relapse; and clonazepam may be associated with fewer depressive recurrences than neuroleptic treatment.

Treatment is initiated with lithium, and, if necessary, tryptophan is added to enhance lithium response; drug-resistant patients who fail to respond satisfactorily to lithium or lithium plus tryptophan are given adjunctive clonazepam therapy. Results are reported of double-blind, controlled studies comparing clonazepam with lithium and with haloperidol in the treatment of acuy manic patients who were newly admitted through an emergency room. The advantages of clonazepam over standard neuroleptics are its rapidity of action, its lack of toxicity, and, particularly important, the fact that it does not cause tardive dyskinesia, a potentially disabling neurological side effect of neuroleptic treatment. For the maintenance treatment of patients with bipolar illness, a combined approach is outlined that involves medications with different mechanisms of action. This is a report on the efficacy of clonazepam in the treatment of acute mania.
