Procedures and treatments from Great Ormond Street Hospital on epilepsy surgery.

Sometimes it is apparent in a few weeks that an operation is needed and this may go ahead quickly. More usually the investigation and decision making process takes between six and nine months. Children move through the decision-making process as quickly as is needed for their clinical condition. For other children the decision process may take up to a year, especially if it is a complex decision or the seizure profile changes.

Many patients selected to undergo epilepsy surgery at our center are offered what we call a two-stage procedure. People with medically refractory epilepsy who have mesial temporal lobe epilepsy can be offered up to a 95% chance of complete seizure control with a single surgical procedure called an anterior mesial temporal lobectomy (or sometimes referred to as amygdala-hippocampectomy.) Also, people with a brain lesion, such as a benign tumor or vascular malformation, that is determined to be responsible for their epilepsy, can similarly benefit from removal of the lesion.

At a median of 14 months after SLAH, ten out of thirteen patients achieved meaningful seizure reductions, while seven were free of "disabling seizures." This included six out of nine patients whose epilepsy was caused by an abnormality called mesial temporal sclerosis. Most important, the procedure was effective in reducing or eliminating seizures in patients with MTLE.

Although articles from 24 centers reporting on 1,952 anterior temporal resections, and from eight centers reporting on 298 localized neocortical resections, met rigid criteria for inclusion in this review, all were considered to be only class IV evidence by the AAN Quality Standards Subcommittee (QSS), because none had a masked outcome assessment.
